After you have scheduled an appointment, you must register on the link that is sent to your phone or email – or your appointment may been to be rescheduled. Our clinic registration is done electronically and we do not have paper forms.

Medical History (Please complete prior to appointment)
Photo Identification and Insurance Card
Medical Coupon (if applicable)
Authorization (if required by your insurer)
Your primary care provider must provide us with insurance authorizations prior to your appointment time.
Payment
Co-pays are to be made at the time of service. For those without insurance, payment is expected at the time of service.

Important Notice

Please do not wear perfume, cologne, aftershave or other fragrant sprays, gels or lotions. Some staff and patients are sensitive or allergic to fragrances.
